Photo Consent and Usage Agreement

Weekly Report Photos: At Kyle's Karate, we take photographs during our classes. Photos of the children are taken for weekly reports, which are emailed exclusively to their parents or guardians. Promotional Photos Consent (Opt-In Available): By registering your child with Kyle's Karate, you are granting permission for the use of photographs taken during classes for weekly reports. If you would like to grant additional permission for the use of these photographs for promotional, advertising, and marketing purposes in a respectful and positive manner, please select the "Opt-In" box below. These promotional uses may include our website, social media platforms, and printed flyers to promote our services. Opting in is entirely voluntary and will not affect participation in our classes.

Registration and Injury Release *

By clicking "I Accept", I acknowledge I am over the age of 18 and have read, understand, and agree to the policies, including the injury release below. Policies: "This is an auto-enrolling program. Your child will automatically be enrolled for each subsequent session of the school year unless you give notice of cancellation to either Bethesda Country Day School or Kyle’s Karate via Email. Email is Kyle’s Karate’s method of contact and all important updates, changes, and other information will be sent to the email address(es) you provide. Although it is rare, in the event that a class is cancelled due to unforeseen circumstances such as scheduling or a sick day, Kyle’s Karate will schedule a makeup time and day with the school and send an email notifying you of the change. Makeup days/times only apply to cancelled classes, Kyle’s Karate is not able to hold makeup classes for students that were absent. In the event of closures due to weather or pandemic-related lockdowns or shutdowns, Kyle’s Karate cannot guarantee all cancelled classes can be made up." Injury Release: “Participation in sports can be inherently dangerous and it is impossible to ensure the safety of all participating individuals. Although many risks can be avoided, I understand that not all risks can be. I have enrolled my child in the personalized fitness martial arts program offered through Kyles Karate. I recognize that the program will involve physical activities including, but not limited to, various fitness activities. I hereby affirm that my child is in good physical condition and does not suffer from any known disability or condition which would prevent or limit his/her participation in this exercise program. I acknowledge that my child's enrollment and subsequent participation is purely voluntary and in no way mandated by Kyles Karate. In consideration of my child's participation in this program, I hereby release Kyles Karate and its agents from any claims, demands, and causes of action as a result of my child's voluntary participation and enrollment now and in future classes. I fully understand that my child may injure themselves as a result of enrollment and subsequent participation in this program and I hereby release Kyles Karate and its agents from any liability now or in the future for conditions that my child may obtain, including any and all injuries, illnesses, or other conditions, whether known or unknown, that my child may incur.”
